When to see a doctor, not a physiotherapist

Physiotherapy is not the right first step for everything. Seek medical assessment promptly if any of the following apply to you:

  • Swelling of the lips, mouth, tongue or throat, a tight throat, wheezing, severe difficulty breathing, or sudden faintness or collapse after a vaccination is a medical emergency. Call for an ambulance straight away.
  • Chest pain or a tight feeling in the chest, a fast, fluttering or pounding heartbeat, or severe breathlessness in the days after a vaccination needs emergency care, not exercises.
  • New numbness, pins and needles, or weakness in the arm or hand after the injection, or not being able to lift or move the arm properly, should be checked by a doctor before you do any exercises. This can point to a nerve or shoulder injury rather than ordinary soreness.
  • Sudden or very bad shoulder or upper arm pain in the days or weeks after an injection, far worse than normal soreness, especially if the arm then feels weak, needs a doctor's review rather than exercises.
  • Redness at the injection site that spreads outwards, with skin that is hot, painful and swollen, or an arm or shoulder that has changed shape or is badly swollen, needs to be seen by a doctor the same day.
  • Feeling generally unwell or flu-like, or running a fever, alongside a hot and swollen area on the arm should be assessed by a doctor urgently.
  • Blisters forming over the sore area, pus or fluid leaking from the injection site, or redness and swelling that keeps getting worse instead of settling, needs urgent medical review.
  • A very high temperature, a racing heart or fast breathing, purple patches on the skin, confusion, dizziness, or cold, clammy, pale skin alongside a sore, hot injection site is an emergency. Call for an ambulance.
  • Arm soreness that is still severe after about a week, or that is getting worse rather than easing, should be reviewed by a doctor before you continue with exercises.
