Understanding Post C Section Core Activation Phase 1
After C-section, rebuild core carefully. Phase 1 uses gentle transverse abdominis work. This deepest abdominal muscle runs horizontally like a corset.
Healing varies by individual. Factors include scar healing, overall fitness before pregnancy, and any complications. Get an in-person assessment for safety.
Gentle activation builds deep stability without strain on healing tissues. It prepares body for later phases like adding resistance. Check with physiotherapist before starting.
Why Focus on Transverse Abdominis After C-Section
Transverse abdominis (TA) provides core stability for movements like lifting baby, bending, or carrying groceries. Pregnancy stretches abdominal wall. Hormones like relaxin loosen tissues.
Early gentle work restores TA function without stressing scar. Physiotherapist teaches how to feel it engage.
Gentle Exercises for Phase 1 Core Activation
Start lying on back with knees bent, feet flat. Place hands on lower belly to feel movement. Breathe steadily through nose and mouth.
Diaphragmatic Breathing: Lie back, one hand on chest, one on belly below navel. Inhale through nose, let belly rise while chest stays still. Exhale slowly, draw belly button towards spine without holding breath. Physiotherapist places hand on lower belly to confirm gentle draw-in, no strain.
Pelvic Tilts: Knees bent. Inhale to relax. Exhale, tilt pelvis to press lower back flat into floor. Hold 3-5 seconds. Add TA draw-in: feel deep corset tighten. Release on inhale.
Heel Slides: Exhale as you slide one heel away along floor, knee straightens slightly. Draw belly in to keep back flat. Slide back. Alternate legs. Keep movement small, 10-20 cm.
Seated March: Sit tall on chair edge, feet flat. Hands on belly. Exhale, lift one knee 5-10 cm while engaging TA. Lower slowly. Alternate. Use wall or chair back for support if balance wobbles.
Four-Point Kneeling Activation: On hands and knees, wrists under shoulders, knees under hips. Exhale, draw belly in without arching back. Hold 5 seconds. Progress to lifting one hand forward, elbow bent, while holding core.
Side-Lying Leg Lift: Lie on side, knees bent at 45 degrees. Bottom arm under head. Exhale, lift top knee 10 cm, keeping hips stacked and core engaged. Lower slowly. Do 5-8 per side. Switch.
Pair with short doctor-cleared walks, 5-10 minutes. Build to 20 minutes.
Phase 1 Core Rehab for New Mothers: What to Expect
Phase 1 focuses on rest, awareness, and gentle activation. Physiotherapist assesses posture by watching you stand and sit.
Sessions include scar massage: physiotherapist uses light circular motions with fingers, moving away from scar line towards hips or ribs to reduce tightness and improve glide.
Homework: Practice 2-3 times daily, 10 minutes each.
Breathing Exercises for Core Postpartum
Correct breathing forms base of Phase 1. Postpartum, many mothers use shallow chest breaths from pain, which weakens TA activation and pelvic floor.
360-Degree Breathing: Lie or sit. Hands on belly, ribs, and low back. Inhale deeply through nose to expand belly first, then ribs sideways, then back.
Sniff-Exhale: Sit tall. Quick sniff through nose, then exhale with long ‘sss’ or ‘shh’ sound like soothing baby. Draw belly to spine on exhale.
Balloon Breathing: Imagine belly as balloon. Inhale to inflate evenly. Exhale to deflate fully, adding TA engagement. Practice before meals or feeds to reset pattern. Looking for a Physiotherapist in Jaipur? Our team in Vaishali Nagar, Jaipur can assess you in person.
Physiotherapist uses real-time ultrasound to show TA thickening on screen, or places hands for feedback. Practice 5 minutes thrice daily.
Safe Abdominal Exercises Post Caesarean: Transverse Abdominis Focus
Post C-section, safe exercises isolate TA before superficial abs. Supine start protects scar.
TA Test: Lie back. Cough lightly. Good TA: midline stays flat, no doming. Poor: bulge appears.
In standing: Feet hip-width, knees soft. Exhale, engage TA, then shift weight side to side without leaning. Hold 5 seconds per side.
Wall Push-Outs: Stand back to wall, feet 30 cm forward. Exhale, press lower back into wall using TA, not butt clench. Hold 5-10 seconds, 8 repeats. Read more about our Posture Correction.
Physiotherapist monitors for compensations like neck strain or rib flare. Integrates pelvic floor: lift gently on exhale with TA. Stop if scar pulls.
Postnatal Core Strengthening in Jaipur: Safe Progression
In Jaipur’s heat, stay hydrated, exercise in cool room. Progress when Phase 1 automatic: TA engages on exhale without thinking. Add holds during function like dressing.
Physiotherapist tailors to lifestyle: holding toddler, cooking, or driving. They use clinical Pilates for controlled moves. Sessions build from activation to integration.
Common Mistakes to Avoid in Early Postpartum Core Work
Wait for 4-6 week doctor clearance. Avoid exercises causing pain, doming, or scar pull.
Ignore form: Use mirror or video to check no bulging.
Hold breath: Always exhale on effort to protect core.
Overdo reps or hold time: Start low, 3-5 seconds, prioritise quality over quantity.
Skip rest days: Alternate active days with light breathing only.
Forget pelvic floor: Pair every TA draw with gentle squeeze and lift.
Neglect scar work: Massage daily if physiotherapist advises, using oil for glide.
Rush progression: Reassess before adding sitting or standing.
Stop if sharp pain, heaviness in pelvis, urine leakage, or scar redness. Seek physiotherapy immediately. Drink water often in Jaipur summers.

Pain, swelling or tenderness in one calf or thigh, which needs same-day medical assessment
Sudden breathlessness, chest pain or a new cough, which needs emergency care
Breathlessness that does not settle, that comes on when you lie flat or that wakes you at night, or a pounding heart with swelling in both ankles, which needs medical review before any return to exercise
A severe headache that painkillers do not shift, blurred vision or flashing lights, being sick, pain just below the ribs, or sudden swelling of the face, hands or feet, which needs urgent obstetric review even if your blood pressure was normal throughout pregnancy, and emergency care if there is a fit or seizure
A sudden inability to pass urine, passing only small amounts, or not being able to feel when your bladder is full, which needs same-day medical assessment even when it is not painful
New numbness around the saddle area, new loss of bowel control, or new weakness in both legs after a spinal or an epidural, which is an emergency
Thoughts of harming yourself or your baby, or feeling frightened, confused, or unusually elated and not needing sleep, which is a medical emergency and needs urgent same-day help
Pain that the painkillers you were sent home with are not controlling, or pain that is getting worse instead of easing
Pain following a significant fall, road accident or other major injury
Fever, chills or feeling generally unwell alongside the pain
Weakness that is getting worse, or numbness that is spreading
If you lose control of your bladder or bowels, develop numbness around the groin, or have chest pain or breathlessness, treat it as an emergency and go to hospital.
Safety limits and when this does not apply
Always follow the advice of the doctor who looked after your delivery. Where this article and your doctor differ, follow your doctor.
Abdominal loading is built back in stages as you feel able: gentle breathing and deep abdominal work first, with sit-ups, planks, crunches and high-impact exercise left until much later. Avoid straining on the toilet and heavy lifting in the meantime.
Do not massage, stretch, foam roll or exercise a calf or thigh that is painful, swollen, warm or red, and do not exercise while you have a temperature or feel unwell, until the cause has been checked.
The gap between the stomach muscles is checked before any curl-up or crunching exercise is added, and a general article cannot do that check for you. If the gap is still obvious around eight weeks after the birth, see your doctor, who can refer you to a physiotherapist.
Electrotherapy and laser are not applied over a caesarean scar, and scar massage is not started until the wound is fully closed and dry.
