ACL Surgery Knee Extension Stuck: Patella Mobilisation

ACL Surgery Knee Extension Stuck: What It Means

After ACL surgery, the knee may not straighten fully. This is knee extension lag. The knee stays slightly bent at rest. Swelling presses on joint surfaces. Pain triggers muscle guarding. Quadriceps weakens fast without use. Surgery trauma tightens joint capsule and soft tissues around the knee.

Knee extension lag happens often in the first weeks. If not addressed, scar tissue builds inside the joint. Early, gentle movement keeps tissues sliding. Full straightening supports normal walking, stair climbing, and balance. Without it, gait shifts forward, loading hips and lower back. Always get clearance from your surgeon before starting.

Untreated lag drags out full rehab. Start with gentle range of motion work. Progress to strength training as swelling drops.

Role of Patella in Knee Straightening

The patella is the kneecap bone at the front of the knee. It works like a pulley for thigh muscles to straighten the knee fully. After ACL surgery, swelling thickens tissues around the patella. The kneecap gets stuck in its groove. This blocks the last degrees of straightening.

Patella mobilisation frees its movement. Shift it side to side or up and down. Do this at home without tools. In clinic, a physiotherapist applies steady pressure while you fully relax the leg. At Trivayu Physio Center, Dr. Hritika Choudhary handles cases where the patella resists home efforts.

Safe Patella Mobilisation Techniques at Home

Use these from weeks 1-4 post-op. Confirm with your surgeon first. Stop right away if pain increases or swelling returns. Choose a firm bed or floor surface. Warm the knee with a warm cloth for 5 minutes to loosen tissues. Looking for a Physiotherapist Near Me? Our team in Vaishali Nagar, Jaipur can assess you in person.

Towel Glide:

  • Lie on your back with the knee as straight as it goes.
  • Place a small towel roll just behind the kneecap.
  • Push the knee down gently into the roll for 5 seconds, then relax. Repeat 10 times.

This stretches tight skin and tissues over the kneecap.

Quad Sets:

  • Sit or lie with the leg straight out.
  • Tighten the thigh muscle at the front. Press the back of the knee flat to the surface.
  • Hold 5-10 seconds. Feel the kneecap pull upward and centre. Repeat 10-15 times.

The contraction lifts the patella without bending the knee.

Heel Props:

  • Lie on your back. Prop the heel on a towel roll 4-6 inches high.
  • Let gravity pull the knee into a straight hang. Hold 2-3 minutes.
  • Repeat 3-4 times a day. Later, add a light ankle weight if comfortable.

Gravity works without effort, easing joint capsule tightness.

Patella Side Glides:

  • Sit with the leg straight. Place one hand flat on the kneecap.
  • Push gently side to side about 1 cm. Hold each push for 5 seconds.
  • Do 10 reps each direction. Use light pressure only.

Avoid sudden jerks. Ice the knee for 10-15 minutes after each session. Track changes: the patella should move more freely after a week of daily practice. Breathe steadily during holds to stay relaxed.

Daily Home Exercises for Knee Extension

Combine these with patella work. Aim for 3-4 sessions daily. Begin with fewer reps to build tolerance. Use a mirror to check knee alignment and avoid twisting. Stop if the knee buckles or heats up.

  1. Straight Leg Raises: Lie on your back. Tighten thigh muscle first. Lift the straight leg 6 inches off the surface. Hold 5 seconds, lower slowly. 10 reps per set.
  2. Prone Knee Hangs: Lie on your stomach with the knee hanging just off the bed edge. Relax completely for 5 minutes. Feel the gentle pull on tight tissues.
  3. Seated Knee Extensions: Sit on a chair edge with feet flat. Slide one heel forward to straighten fully. Hold 5 seconds at the end range. 10-15 reps.
  4. Wall Slides: Stand with back against a wall, feet 12 inches away. Slide down to a 20-30 degree knee bend, then push up to straight. 8-10 reps.
  5. Terminal Knee Extension: Stand holding a chair for balance. Focus on straightening the final 10-20 degrees from a slight bend. Hold 5 seconds. 10 reps.

Dr. Ishu Sharma at the clinic adjusts these for your exact lag angle. Measure progress weekly with a simple phone app goniometer or by comparing to the other knee.

Stiff Knee After ACL Surgery: Home Rehab for Knee Extension

Stiffness often peaks between weeks 2-6 after ACL surgery. It feels like a frozen knee. Pair extension work with bending to avoid one-sided tightness. After a heel prop session, sit on the bed edge. Lift the foot to bend the knee to 90 degrees. More on Knee Pain Treatment and how we assess it.

Another move: slide the heel up the bed to bend, hold 10 seconds, then slide back to straight. This pumps fluid through the joint and nourishes cartilage. For more bend, try prone flexion. Lie on your stomach.

Days 1-3 post-op: focus on tolerance with short holds. By week 2, increase reps if soreness fades within 24 hours. Wear a compression sleeve during short walks to control swelling. A physiotherapist checks passive extension (they move your leg) versus active (you move it). The gap shows muscle weakness.

ACL Post-Op Stiffness: Knee Bending and Straightening Exercises

A stiff knee after ACL recovery needs balanced bending and straightening. Imbalance causes patella tilt or quad shutdown. Alternate cycles: straighten fully with quad sets, then bend halfway. Example routine: 10 heel slides up to 60 degrees bend, straighten each time. Follow with 5 prone hangs for extension.

For advanced home rehab, use a foam roller under the thigh while seated. Roll slowly from hip to knee to loosen hamstrings. Then do seated heel slides with a sock on a smooth floor. Push to full straight after each bend. Do this on even ground to mimic walking. You may also find ACL Surgery Knee Extension Stuck at Home useful.

Track daily: note bend angle (aim for 90-120 degrees by week 4) and straight angle (0 degrees). Use a yardstick against the shin if no goniometer. Soreness is normal if it settles overnight. Elevate and ice after walks.

When Home Fixes Aren’t Enough

Monitor for no improvement past 2-4 weeks, sharp catching pain, new swelling, limp, very weak quads, or night pain that wakes you. These point to excess scar tissue, called arthrofibrosis, or muscle imbalance.

At Trivayu Physio Center, 204, 2nd Floor, Om Tower, Gandhi Path Road, Nemi Nagar, Vaishali Nagar, Jaipur, Dr. Jyoti Choudhary leads rehab programmes. We apply Electrotherapy to cut swelling and ease pain. Orthopaedic Rehabilitation rebuilds strength step by step. Also available at Vidyadhar Nagar, Rangoli Gardens.

Preventing Long-Term Knee Stiffness After ACL

Stick to daily exercises without skips. Keep good posture: stand tall, avoid crossing legs. Increase reps weekly as comfort allows. Walk on flat ground 10-15 minutes twice a day using heel-toe pattern. Ice and elevate after activity if any puffiness shows.

Common questions

What causes knee extension to get stuck after ACL surgery?

Swelling blocks joint motion. Pain causes guarding. Weak quads fail to pull straight. Patella sticks in tight groove. Scar tissue forms without movement.

Can I do patella mobilisation at home after ACL surgery?

Yes, with surgeon okay. Try quad sets, heel props, towel glides, side shifts. Gentle pressure only. Ice follows.

How often should I do knee straightening exercises post-ACL?

3-4 times daily. 10-15 reps per exercise. Short bursts with bending mixed in.

When to see a physiotherapist for stuck knee extension?

If no gain in 2 weeks, ongoing pain or swelling rises. Stops scar buildup like arthrofibrosis.

Are there safe home exercises for ACL knee stiffness?

Yes: straight leg raises, prone hangs, seated extensions, wall slides, terminal knee work. Slow start, mirror check, ice end.

How does a stiff knee affect ACL recovery?

Delays normal gait, stresses other joints, builds scar risk. Full extension plus bend speeds safe return to activity.

When to see a doctor, not a physiotherapist

Physiotherapy is not the right first step for everything. Seek medical assessment promptly if any of the following apply to you:

  • Pain following a significant fall, road accident or other major injury
  • Fever, chills or feeling generally unwell alongside the pain
  • Weakness that is getting worse, or numbness that is spreading

If you lose control of your bladder or bowels, develop numbness around the groin, or have chest pain or breathlessness, treat it as an emergency and go to hospital.

Safety limits and when this does not apply

  • Any exercise that sharply increases your pain, or leaves it raised the following morning, is too much for now.
  • Electrotherapy and laser therapy have their own restrictions, including pacemakers, pregnancy and treatment over a known tumour. Your physiotherapist screens for these before use.
